Pages 149-156 are clearly photocopies and have been inserted after the book was bound.  There is no way of knowing whether this was done at the publishers or afterwards, but there is no sign that the missing pages had been torn out.  Occasionally defective books like this are sent out as review copies, and this may be the explanation here.

One of those books that American booksellers ask ridiculous prices for – one is asking £1,694.60 (plus postage!!) for a copy he describes as “very good”. . . .
